KINDERGARTEN REGISTRATION BEGINS IN FEBRUARY THROUGH MARCH
Children turning five years of age on or before December 1st may be registered for kindergarten.
Please contact the registration office at 631-369-6706 between the hours of 8:00am and 4:00pm to
schedule an appointment. Appointments will be available Monday through Friday from 8:30am to
2:00pm. Your child does not need to be present.
Kindergarten screenings will be scheduled by your home school between April and May, only AFTER
you have completed the registration process.
If your child will be attending a private, parochial or charter school in September and you live within
the Riverhead School District, you will still be required to complete the registration process.

The following documents are required to register a student:
(1) Birth Certificate
(2) Student’s Current Physical & Immunization record
(3) Proof of Residency (Landlord affidavit below if applicable)
(4) Photo Identification
(5) Proof of Guardianship (if applicable)
Please refer to the registration application below for further details.
FOSTER CHILDREN ONLY: a guardian or agency must present evidence or have in their possession
a court order assignment and/or a Department of Social Services assignment or agency assignment
(DDS 2999 form) that they have been given authorization to act on behalf of the State of New York.
