Special Education Records Request

Special Education Record Requests

To request Special Education Records please:

  • Click this link for a request form.
  • Record requests must be submitted with a copy of a government issued photo ID (driver’s license, school ID, passport, etc.) 
  • Please allow two to seven business days for processing, and all requests will be processed in the order they are received.
  • You may email your completed request form to or by fax at (631) 591-5980, or by mail to the Special Education Department, 700 Osborn Ave., Riverhead, NY 11901, Attn: Laura Jacques

The Special Education Department does not issue High School transcripts. For transcripts, please contact the High School Guidance Department.

As per New York State Department of Education, Special Education records are retained for 7 years after the student turns 21. Records more than 30 years old may be limited or unavailable.

Transferring to a New School:

When registering at a new school district, notify the registrar that the student is classified for special education. The school district will contact the Special Education Department and the records will be sent via an encrypted email, usually within 24 to 48 hours.
